3. Employer’s Report:

The employer is responsible for reporting the accident to SOCSO. They should fill out and submit the necessary forms and documents to the nearest SOCSO office within the stipulated timeframe. These forms may include Form 34 and Form 34A, depending on the type of accident and injury.

4. Medical Report:

The attending doctor who treated the injured worker should prepare a medical report detailing the extent of the injuries, treatment provided, and the expected duration of recovery. This medical report is crucial for the claim process and should be submitted along with the claim.

5. Claim Submission:

The worker, or the employer on behalf of the worker, needs to submit a formal claim application to SOCSO. This claim form (usually Form 8) can be obtained from the nearest SOCSO office or downloaded from their website.

6. Investigation and Approval:
